Helping people manage high blood pressure

 Tracking your health goals just got easier — even when you’re on the move.

The American Heart Association has upgraded its Heart360.org website  for patients and blood pressure program participants with a new mobile-friendly design.

Originally launched in 2007, Heart360.org allows users to track and share health information with a growing network of nearly 4,000 healthcare providers and volunteer health mentors. It’s an integral part of the association’s Check. Change. Control. initiative that helps people manage their high blood pressure.

“Helping millions of people control their blood pressure is one way we can reach our 2020 goals of reducing heart disease and stroke by 20 percent and reducing deaths from these diseases by 20 percent,” said Patricia C. Lane, M.B.A., B.S.A., R.N., a member of the AHA High Blood Pressure Expert Panel and  Administrative Director of Neurosciences for Bon Secours Health System , Inc., in Richmond, Va. “This online tool will not only help people track their health status, but also help us measure our progress toward that goal.”

The new design allows participants to update and access their health data from their mobile devices and provides easy recall to share with their healthcare providers or volunteer mentor.

Heart360 also educates patients about their condition with the most relevant educational content, tools and resources. Users gain a better understanding of their current health information and are guided to a more specific action plan suited to their current health needs.

Healthcare providers can directly connect with their patients to monitor and assist them to improve their health using the provider portal.

People who have partnered with the American Heart Association through Check. Change. Control. can connect with volunteer health mentors through churches, community centers or local retail establishments using Heart360.org. Volunteers can track participants’ status and send them messages, important announcements and information on local events, as well as local resources.

 Both the provider and volunteer portals of the site are being redesigned into the same mobile-friendly responsive design as the participant portal and will launch in early 2014.
